Most of these ancient "Trans holy priests" never got to choose their gender.
If you were born on certain days, had a weird birthmark, or any number of arbitrary things, the local priests could claim you as "chosen", lop your junk off, and raise you as basically a religious slave girl.
They also sometimes sold or traded their kids to the priests for this. It wasn't a glamorous life. They'd basically never hit puberty, so they'd end up permanently sickly, frail, and "girlish".
