You can't know what will make someone feel uncomfortable, a DM/teacher/whatever you can give a rough warning but if it's something important you need to give them an advanced warning. They can't read your mind, but they can listen and if they're friends will take it to heart. Also saves you from feeling on edge, just waiting for the topic to maybe come up. Makes it easier for the DM too if they don't have to worry about walking on egg shells.

If the players lie at the onset about their fears, an X-card certainly won't help them get out of it when it's there. They might even continue to keep silent while the scene plays out. A casual talk with the DM is much less harrowing than a situation where your blood runs cold and you tremble in fear while everyone else carries on as nothing is happening.

If you can't give the DM your consent truthfully for whatever reason, which isn't consent all, it's best to just recuse yourself and sit the campaign out.
