Cosmetic surgery in general is this wild field where it's lucrative and there's little training or regulatory oversight. You get some weird surgeons working there.

Back in the 70s when GRS was new they were just experimenting on patients in real time. You'd see case reports where it boiled down to "neovagina looks okay, didn't rot away in a couple months like the last one I made."

Oh, they know. Surgeons just don't communicate it to their patients. Only a minority of patients are actually happy with GRS, and if surgeons actually told patients that then, well, they wouldn't have many patients would they?

Most of the research in GRS has been focused on trying to figure out surgical techniques that prevent your stink ditch from just rotting away. Penile tissue is kinda obviously used to substantial amounts of blood flow, and cutting it off results in the ol' dick rot (this is actually a pretty common problem that urologists see...) inverted or not.

Before my heart surgery, I spent a lot of time researching which method I'd prefer, and the differences were minimal enough that I could flip a coin, all things considered, and a year later I'd have been in the same situation whichever choice I made. I just can't fathom why these people don't research the results and consequences of SRS. Why are they always shocked by the shit that goes down? I knew all about the aches and pains I'd be getting, and which ones should and shouldn't worry me.
Do they really rely only on the word of the doctors who have financial incentive, and the only circle of insanity that in Troonworld?

They can develop an aversion to their penis that is so strong that they want to get rid of it at all costs. Their obsession grows to the point where it is distressing every time they are reminded of their genitalia. That level of sexual dysphoria grows ever stronger as their obsessions are fed. They begin to develop opposite sex sexual characteristics, which probably in all cases leads to increased dysphoria with their native sexual characteristics. That is why you do not treat gender identity disorders with hormonal or surgical treatments except as a last resort, and only after several years of intense therapy, and never ever should children be subjected to that.

Nowadays that kind of scrutiny is not happening like it used to, you can go out and get cross-sex hormonal treatment in mere weeks. A better place to start treating GID would be to remove access to pornography from sufferers, disconnect them from the transgender internet echo chambers, and work at breaking their obsessions, work at ameliorating their autogynephilia by advising against crossdressing.
