This isn't even remotely how insurance works. Insurance sets reimbursement rates, and doctors can take it or leave it. Really big medical centers can negotiate with insurance companies to increase their reimbursement, but sole proprietors and small clinics either suck it up or don't do business with cheap-ass insurance companies.

Cosmetic surgeons of any kind generally don't take insurance because they don't have to. They do just fine with cash patients. This is why most cosmetic surgeons that are willing to work with insurance companies at all have high rates of being butchers. They take shitty insurance reimbursements because rich people aren't stupid enough to go to them. This dude is lucky Kaiser found a single dick chopper to contract with.

Jesus fuck that is... yeah. And still the primary concern is "gaining more depth" ... fucking seriously? What did he honestly expect the ER doctor to do anyway? A vaginal fistula is certainly gross and distressing but it's not an emergency, though it's pretty telling if the area looked so bad they were concerned enough to keep them overnight and consult the fuckin' trauma surgeon.
