They all look like that. It takes a while before they turn normal again but once in a while you have complications where they must be "revised" aka removed entirely. Some troons skip the risk altogether and just go nipless, and some doctors charge extra for nipple retention so it's cheaper. Even if they don't fall off it can take years for the sensation to return and often it comes with nerve damage described as sharp shooting pain which may be so intense that they choose to have them removed after all. Fun times.
