It seems like it should be less common, because high testosterone is a big reason a person gets acne. Testosterone makes your skin produce more oil. Couple possibilities here

1) The testosterone blockers aren't doing their job
2) These people aren't washing their faces

3) They eat absolute garbage, because, typical of troons, they have no traditionally "feminine" interests--such as cooking and nutrition.

Yeah the story makes absolutely no sense. Why would anyone care if you play female or male avatar, let alone jump to tranny conclusion? Guys choose to play as ladies all the time for various reasons like better voice acting or more pleasant bum to look at 40+ hours.

If this actually happened, and his mom's first impulse really was to jump to tranny conclusions, it's probably because he's already been caught engaging in cross-dressing, and maybe consuming tranny porn. Has he used his sister's clothing in order to cross dress and get off? Or even his mother's? We don't know because he doesn't say. Autism as a hyper-masculinization of the brain is an increasingly well supported theory. Thus the giant chins and neanderthal brows, if said endocrine environment applies to stuff besides the brain and assuming it's due to that environment rather than problems with certain receptors. Then despite having chiselled butt-cleft chins that go on for miles, they think they pass because autists struggle to identify subtle differences in facial features in the first place.

Humans actually all look pretty similar, objectively. We struggle to identify differences in similarly complex objects to a human face. We're just awesome at faces because there's a whole huge portion of our brain that's evolved to help us tell each other apart. It doesn't work to well with autists though.
