I'm pretty sure his injection is some sort of antipsychotic drug. According to Nick, it keeps his "mental videos" in check. What that means is that if he's not medicated, he's constantly fantasizing about committing murder, rape, child molestation, torture, and cannibalism. Believe it or not, the Nick we usually see is Nick at his most sane and least depraved.

I don't recall his Twitter and I don't even really want to see it, so I don't know what he's saying, exactly, but I think that as long as he's not making specific threats, there's not a lot the cops can do about it, unfortunately. Maybe I'm wrong; I'd sure like to be.
