This might not be furry, exactly, but it's definitely awful (if impressive) and feels like a condensed version of what going through this entire thread feels like. The self-mutilation is an extreme masochism thing; you see the same sort of material from people who fetishize castration or knife-fucking in general as the ultimate sexual experience (I guess because it's so destructive/traumatic?).
There could also be a transgender bent to it as like an extreme "solution" to gender dysphoria, but I think that's probably down to the commissioner; the author apparently is open to any gender on that particular commission.
