From what I've heard from people who do it, the orgasms are quite qualitatively different. For those that don't know, Japan has an extremely medicalized concept of transsexualism. They are, essentially, "truscum", and believe transsexualism is a neurological disorder that makes be misaligned with your body parts. Both psychiatric professionals and the legal system do not jive with the "identity" concept of transgender, but it is worth noting there is a counter-push from some Japanese activists (I would presume due to the same reasons it happened here).

No one in Japan is forced to get SRS. It's just that their requirements for legally recognizing transition are much more stringent. I'd say it's a good thing if it discourages people from transitioning because SRS is absolutely inhumane. People who legitimately have gender dysphoria still have the option to medically transition, but shouldn't this be a last ditch option only to be undertaken when all other treatment options have failed?

There actually used to be similar requirements here in Canada, but these days it's trivially easy to legally transition. To change your sex on your birth certificate you only need to fill out a form, get it signed by your gender-affirming doctor (because all of those nasty "gatekeeping" doctors here have either been fired or intimidated into playing along) and send it to the appropriate service centre. Under self ID, it doesn't matter whether you're actually medically transitioning or not. I know one dude out in Alberta who did this to see if he'd get lower auto insurance rates as a woman (spoiler alert: he did). This is also how we've enabled lunatics like Yaniv to terrorize women and young girls in bathrooms all over British Columbia.
