That's exactly what classic pill mills were. The opioid pill mill craze was the same exact business model as an informed consent clinic, but at a larger scale with the sole motive being money. West Virginia had a ton of infamous ones where millions of opioids were shipped into a town with only a few thousand people. Cars were lined up for blocks to get opioids from a dinky pharmacy with a drive thru window that literally had "it's all in the bag" as a slogan. The business model was the exact same, walk on in with a wad of cash and walk out with a prescription for oxis. One businessman took it to the logical extreme and ran both a pill mill and the pharmacy that took those orders.

If you want an in depth look at an opioid pill mill, this article is a classic. It's about one pharmacist who took down a pill mill after he kept getting oxi prescriptions from the same doctor. When he pressured people about this he learned that lines were insane and that the "clinic" was open until 4-5 in the morning. When one of his customers died after he saw the pill mill for himself, he went hard after them.

Informed consent clinics or HRT pill mills are different. There aren't long lines for it, the clientele oftentimes can't afford to drive out of state, the side effects are different, and politically it's on the right side of history. If you deny HRT to your customers you can expect the trans lobby to nail you for being a literal bigot and corporate will play along with the request. Plus people aren't dying right away from ODing on estradiol and spiro so there's less of an urgency to go after them. Instead they die years later from a David Reimer style suicide when they realize they've ruined their body based on something they saw as trendy on social media.

We keep running into estradiol valerate shortages. It ain't women who are using it all, I can tell you that.

Realistically the reason why IC clinics took off is that it's a very easy case load. An endo can either deal with brittle diabetics whose blood glucose looks like a roller-coaster of death or they can just deal with troons who want their titty skittles. Maybe you see a prolactinoma in them every once in a while but it's not really that big of a deal. Managing other endocrine conditions without accidentally killing people is much more difficult.
