In the US, diagnosis isn't really standardized for most disorders. There isn't consistent paperwork or codified requirements, for example.

On the other hand, the prescription system is very standardized. But if a reason needs to be given for a given prescription, it's pretty much just comes down to the doctor's word (going back to the non-standardized diagnosis) that it'll help for whatever ails the patient.

This is why doctors are able to prescribe puberty blockers, for example. There is no such thing as a designated "puberty blocker" to start with; what they're prescribing are other, skeevy cancer drugs that are being repurposed. It only rests on the doctor's word that they'll do what they claim it to.

If a doctor started prescribing, idk, opiates to randos for headaches, someone could file a complaint with the medical board and eventually they'd probably decide that that's unusual enough to suspend their license or whatever. But there's no hard and fast rule that says "no, you can't prescribe opiates for headaches".

With informed consent, it's not really consistent how it's implemented. Sometimes they do have you meet with a shrink who asks you a bare number of questions. Sometimes you just meet with the endocrinologist and tell them your therapist says you're totally trans, then they write you the script. Maybe they'll give the therapist a ring, maybe they won't.

I think Chris Chan met with a shrink briefly, who referred him to an endocrinologist at UVA, who promptly wrote him a script for titty skittles.
