If they’re having the combination of an acute haemolytic crisis plus pneumonia, That’s a double whammy. Haemolytic crisis with less lung damage= the ones with normal lung sounds and low spO2. Both together are the ones in deep trouble.

Have you tried just shoving a few units into any of them, and following up with iron and folic acid? - could be justified if RBCs are really low.

Yeah I've only seen like two cases of favism in my career.

I'm uncomfortable ordering a transfusion because our blood stocks are already low and I don't think there is much forthcoming. Next case I see is getting methylene blue though. I think that may help.
 
Has anyone tried blood transfusions as an approach? I’m going back to that first JAMA case series and looking at the labs. The red cells are wiped out, just nuked. These patients are having an acute crisis, over an hour or two that’s ending up with them on vents and really high pressure is being used, often to no avail.
I wonder if at least part of the issue is the blood side of the equation. Is that rapid decline an acute haemolytic crisis?

It can worsen crisis in patients with g6pd. (? Or do I misremember that? Something to do with insufficient NAPDH?) But would certainly be worth trying if they were lower risk and high metHb.

Thinking about this as partly a problem of redox states throws up some interesting avenues of exploration, treatment wise ...

Yes they’ve seen car wrecks when they rotated through the ER years back, but if you go through rotations and end up in a gastro or dermatology or genetics or research (ahem) line of work you’re totally removed from the drama, the death, and the gore.
I have a family friend who is a retired orthopedic surgeon. He grew up in the city and was assigned an ER rotation out in the sticks. The one that freaked him out the most was a 'brush hog' incident. Mostly because how completely mangled the guy was, but also because this was an area of the US with feral hogs. He refused to go in the state park for 2 weeks until he finally asked one of the local docs what a brush hog was.

It's a tractor mounted rotary mower. The guy fell off the back and the mower ran over him.
 
Polish numbers: 311 new cases, a mere 7,6% growth, but also a huge drop in daily tests. Whatever, it's still just hiccups after that change in daily reporting. I'm guessing between 350-400 confirmed cases and 6000-7000 tests for tomorrow. But that's not the main point of interest. Everyone was talking about May today.

You see, there's a presidential election coming up. The president is not exactly the most powerful politician in our system, but it's good prestige and if your government is already struggling, he can fuck you up for good with constant vetoes. So the government really wants a quick election because their guy is well liked and most likely to win. Most opposition parties really don't want the election because they will get their asses handed to them. Regardless of intentions, having people crowded around ballot boxes is suboptimal in these conditions, so the current idea is to have a mass mail-in vote, citing Bavaria as a recent example. This promises to be a guaranteed failure; the national post office can't handle the task and many people will refuse to participate because they think holding this election is a threat to democracy. Alternatives being floated so far include proclaiming a state of natural disaster which would postpone any elections potentially indefinitely, or amending the constitution to extend the presidential term. Go figure.

Shame about BoJo, I hope he can recover but I hear prognosis is poor once you're on intensive care.
Just buy the electronic voting system from the baltics and you're golden.

These are those very circumstances. It reduces Methaemoglobin to haemoglobin. The kicker is that there’s a very common enzyme deficiency in humans which means people are missing a bit of this specific chain and if you use it in them you make it worse.

Oh right my bad. I forgot they can only stick around so long as their party/coalition allows it correct? Even so you're supposed to have an election every 4 or 5 years right? And no one since Blair has held office for more than the usual turnover correct?

New Zealand cases now over 1,000. Despite the lack of testing, cases continue to increase steadily, 82 cases being added yesterday. Aunty Jacinda is claiming that the fact that the Ministry of Health is too incompetent to test widely or identify the origin of cases (leaving their cause of infection 'unknown') means that community transmission isn't happening.

Likewise, they are touting the fact that Google's stalking software shows that people are leaving the house less as a success. But not, of course, actually looking at known carriers and those who have returned from overseas and making sure that they don't wander around infecting people- the sort of thing that would have prevented this lockdown from being necessary in the first place. Infected people with New Zealand citizenship (very easy to get) are still allowed to return from overseas and start infecting more people.

Meanwhile, horse face continues to crack down on any forms of recreation that decent working New Zealanders enjoy. Not only hunting, and surfing, but even fishing alone off the beach or a pier is banned.

He's no public health expert but a smart guy with a very solid grounding in statistics (I believe he worked in resource geology) who exposed some very bad statistics on the part of some celebrity climate scientists. He's been reading a lot and looking at the stats and exposes a lot of dumb misconceptions of data that people are falling prey to. He's also talking about what it will take to actually start controlling spread.

This is the funniest one from the last week in my opinion. The incompetence of these people at the CDC and similar organizations and their political bosses (who are the real culprits) reeks of Jacinda's 'community transmission? what community transmission' nonsense that she's still pushing even now. It's like Australia and the US were pretending that community transmission could happen in China to Chinese people but not in Australia to 'Chinese-Australians' or 'Australian-Australians' for.. absolutely no reason at all.

Britian is a country with over 66 million people in, it's densely overpopulated, the government has been very sly and not giving genuine figures about this as they haven't been testing half as much people as they should. This is following the same trend as Italy and is spiking even bigger every day with more deaths.

At least every 5 years according to the Parliament Act 1911. The Fixed Term Parliament Act 2011 means it's hard for a PM to call an early election - you need a 2/3 supermajority in Parliament. The PM can still lose a confidence vote though, that will still trigger an election.

However, look at what happened in WWI and WWII. In each case, Parliament voted to postpone the election until the end of the war. Because Parliament is sovereign but it can't make rules binding a future Parliament lots of things are possible that would not be in a country with a constitution that overrides Parliament.
