It might just be me, but even before the "age regression" label, there were quite a number of fics about a character getting brain damage whether permanently or with some slow recovery. In a way, I get the appeal because depending on the character, you could do some interesting takes with them. Example: there's a popular/famous (I dunno lol) Pokémon story called "Damaged" where Mewtwo got shot in the head, and it basically blew out his psychic abilities to the point he had to learn to walk, talk with his mouth, use his hands, fight physically, and all the like while he lived with Bill who had rescued him. It apparently went into a different direction after a while, but I haven't been able to read it before it apparently disappeared from the Internet so the author could (slowly) rewrite all 40+ chapters. It's like the first of its kind in terms of Pokémon fics.
So I think before it got fetishized/a "fanbase", it used to be treated with respect or in awe. It was probably also considered an "easy way out" in terms of "rewriting" a character or just so the author wouldn't have to come up with an interesting dramatic plot, but something ended up warping it into the creepy factor we see it today. Maybe some Tumblrina got triggered over a fic treating headmates or another mental illness with serious negative implications and went "This is how you're supposed to write it, you ableist!!!" and it snowballed from there.

Castlevania Theory
The Castlevania series stars a family of BDSM dominatrixes and dominatrices who take time machines throughout history to whip the shit out of Dracula to satisfy both their dominance and vampire fetishes. Unfortunately for Dracula, he's not into submission, so he created a really fucking complicated series of puzzles with Castlevania II: Simon's Quest, even ripping out his rib in hopes of confounding the Belmont clan. Because Simon was such a horny bastard, he figured a way past all of it anyway. What a sick fucking douchebag.

The Dan and Phil fan Phandom are very active on A03. There is even a group chat to encourage people to write up oneshots and it keeps getting promoted on Tumblr. There is also a Tumblr blog dedicated to Phanfics posted on that site and also this one.
Also I swear 80% of Queen (band) / Bohemian Rhapsody fanfics are to do with mpreg and/or there being an Alfa/Beta/Omega dynamic and lots of dom/sub undertones. There is a Tumblr blog for that as well. This blog is of this fanfic writer on A03 and they do quick oneshots upon request over ask based on their stories. Their writing on A03 is quite good tbh, but their Tumblr is a hot mess of wtf.
All I wanted to do one day was read fics of Freddie not dying and then I fell into the trap of...this.

Granted, I don't think this level of lunacy is exclusive to Jojo, at least on AO3. Take any IP with a large fandom, and you're almost guaranteed to get some bizarre fetish smut within the first few results.

I love it when people like this have to go "DO N O T SEXUALIZE THIS!" as if 1. It's going to stop people from flipping them off and spending the evening writing a 20 page BDSM fic and 2. That fanfiction is for children. You have no idea how many people on AO3 and Tungle like to bring up about kids in the fandom, which is ESPECIALLY hilarious in the Jojo fandom.

I love the purity culture currently going around in the JoJo fandom. It's gotten to almost ridiculous levels. People act like they can police fandoms that have been around longer than they've been alive while at the same time writing and consuming this uwu bullshit and they convince themselves that since no dicks come out it's not sexual.

I remember a while back about a pretty mediocre "Giorno becomes a vampire" fic where the author was VEHEMENTLY pasting "We need a platonic Abbagio week (because she was posting this at the same time it was happening) because this is PEDOPHILIA and if you like it you're a PEDO SUPPORTER" or something to that effect. The comments was a really amusing slap fight between the author and people saying that a platonic ship week was stupid, peppered with the occasional comment of "Nice fic!". That was until I refreshed the page for any new ones and suddenly it was all barren. Gotta love people dishing out criticism but not taking it.

Like I said, all the comments fighting got deleted, leaving only the praises (because of course), but the tags still make me giggle. I regret now not screenshotting everything as it quite the amusing thing.

Jojo AO3 is either filled with people like this, the soft bois or the people only in the fandom for writing niche fetish porn. I wish all the other franchises I liked had such people like these more just so I can stop talking about Jojo so much. Fortunately, they're mostly untouched.
