I'd take that as "huh, not bad, that could pass as weird looking female junk at a glance if it wasn't attached to an obvious fucking dude." Most of the "good" results look ok when you're just scrolling past the image but it's only when you're morbidly curious enough to stop and give it a good look can you tell how wrong it is.

Of course the doctor was doing a swab, most medical people have some level of morbid curiosity, he wanted to know what precisely the fuck might be growing in there. I suspect the microbiome is similar to the nasty crap that pools in the bottom of dumpsters and garbage trucks in the summer.

Teens coming up with whacky new gender identities has been a thing for a while now, the oldest evidence I can have of them is from 2014. Here's a whole blog collecting them and mocking them, if you're interested in that kind of content.
They were pretty popular on Tumblr from 2013-2016 iirc. I even had an irl friend come out as "stargender" during that time, and another who came up with her own label of "oruborus gender, a gender which consumes itself like the oruboros."
The xenogenger kids got a fair deal of mockery on Tumblr, mainly from the self proclaimed "anti-sjw" crowd. They were very popular among young teens on the site, but like other Tumblr trends of the era like headmates and kinning, seem to have faded over the years. Current Twitter houses a lot of trends from 2016 Tumblr, so it makes sense we're seeing a resurgence of whacky gender identities there as well.

Oh yeah, I remember reading about this fuckery years ago. Its popularity may have waned on Tumblr, but you'll find plenty of gender insanity (along with self-dx and headmate bullshit) on Twitter these days. I don't know what it is about the latter that makes it so much more insane than the former in so many ways.

You're missing the point. Their entire goal is to make "biological sex" a meaningless term. All that matters to them is gender.
I'm not sure why they're trying to do it, but I think it might have something to do with how progressives embrace a variant of the tabula rasa theory. Since everything is a social construct and therefore malleable and changeable, any innate aspects of the human being (sex, ethnicity, mental ability) must be willfully ignored as hard as possible.
