Idk I think it's completely possible for some doublethink to exist there, not all people with a mental illness are stupid or 100% delusional about it, sometimes it's just an impulse control problem more than anything. To use your ED analogy think of binge eaters or bulimics, they really don't fucking want to binge and are ashamed of it and hate themselves while it is happening, but resisting that urge is next to fucking impossible for them, from the outside it is easy to say "well don't fucking do it then" or "don't think about it" but if it were that simple it wouldn't be a problem.

Men have led normal lives while crossdressing and hanging out in seedy gay/fetish bars on the weekend in secret, keeping the AGP fetish stuff to appropriate times and places without letting it interfere much with family or work life. We used to just call them transvestites, and it was a bit of a joke, but generally people kept that shit private (a good example of this was that dude on Hoarders whose entire family was fucking shocked and mortified when they found his lingerie/porn/dildo stash). It wasn't until the last 10 years or so that the troon shit got out of control and more are openly declaring it their whole identity, forcing people to participate in their perversion, and reinforcing it constantly until it becomes a full blown delusion rather than just an unusual sexual proclivity. With the blessing of medical doctors and psychiatrists, which is the really fucked up part. I don't even get why feminizing hormones are given to men, it just raises hell with their entire system for not one iota of physical difference aside from getting the weird eunuch fat distribution and moobs if even that. Masculinizing hormones for women makes a bit more sense (not that it's healthy but by trans logic) since facial hair, increased musculature and male-patterned baldness means they might actually have a hope in hell of passing if you don't look too hard. SRS only produces semi-okay results very rarely and even then it's just a crude approximation with all kinds of complications and questionable function at best and I don't know how it's even fucking legal.

Since this clearly came out wrong let's try again to get my point across. Agreed, boobs are pretty amazingly moldable and this allows faking without too much trouble. Same chest can look completely different with right bra shirt compo. Especially in pictures you can fake few cups pretty dam easy. I have fairly large pair so usually more conserned to make the girls look smaller but same rules ably. Stronger cleavage, higher position, badding and long visual lines from neck to boob make them look larger. Contoring can also give nice illusion of depth if you want to go extra mile.
