Or stupid. It's rare, but it being a surprise can be understandable, for women who don't show or have any reason to expect it. If 'I Didn't Know I Was Pregnant' is any indication, though, stupid is common.
Also when troons are involved, stupid is Occam's Razor. Considering they already think they can change biology with magical thinking, the really thunkful part as mentioned earlier is how people who you can murder by saying their female name can bear to be pregnant and give birth, which by their logic would be the most dysphoric thing imaginable.
To be fair, very few troons have done it - except they want those few FtMs to be enough to change the entire language around childbirth and child-rearing. It's really noticeable how the biggest changes both sets of troons want to enforce have the most impact on women...

Bit late, but for some in more liberal areas theres also a conscious or unconscious "see no evil, hear no evil" element. Theyve spent years knowing fags and drag queens that arent especially objectionable (I'll gladly admit that a lot of fags and DQs can be quite fun and personable in their own right), and then when trannies came on the scene I think these women were already "fluffed" as it were by DQs. If they dont spend much time in the relevant corners of the net or do a lot of social media or follow/know of the myriad of trans horrors, they just think trannies are another quirky campy flavor of queer. "I know a bunch of trans and they're totally fine people!" Sure... great... MAYbe... But they just hang out in social settings in very trans-accepting areas, with any trans-related conversation just being about how hard it was coming out or understanding themselves or whatever, or about more egregious anti-trans attacks and so forth (being relayed by totally unbiased trannies of course), so that "dark side" doesnt rear its head. And theyre also already in a mindset of "theres nothing wrong with being 'queer' and only ignorant bigots shit on the gays" with said deference being bestowed on trans just by virtue of being 'queer.' But they dont want to dig through their social media to see if theres any "choke on my cock, TERF!" posts, or grooming "hatching" posts, or "its so validating to get my boipussy period after getting skewered for an hour" posts, or "fuck your bigot 6yr old's feelings, I piss where I want, I swing my girldick where I want" posts. They dont feel inclined to broach "should a 15 year old chop off her tits" or "should an 11 year old be pumped full of blockers" or "should there be violent penis in womens prisons" or such topics with trannies that may reveal a more disturbed mindset. Maybe the ones they know are some of the "good ones" (eg, not TRAs, not outwardly misogynist/excessively deviant etc), but if you tell me you know a dozen trannies, I guarantee theres at least some in the bunch that are way more fucked in the head and in their viewpoints beyond "I just want to present how I feel inside and not get harassed." They just know that the trannies *they* know are friendly when in their company and arent registered sex offenders (as far as they know, and to even consider the possibility, let alone check, would be transphobic and literally unthinkable), so those bigots need get over their shit and get on the right side of history. Add in a bit of that "female agreeableness" and the idea that the fundamentals of language and definitions of words dont matter as much as hurting a grown man's (or a confused rebellious teen girl's) fee fees, and you've got a recipe for a thoroughly frustrating enabler.

I’ve seen the argument “but the concept of transgender hadn’t been discovered yet!” when it comes to retroactively labelling people like Johnson as trans, when really back then people who felt like dressing up as the opposite sex didn’t feel the need to fuck up their bodies with hormones and surgery.
