Further more the stereotypical "beer belly" that alcoholics tend to get is not actually caused by weight gain per se, it is a symptom of the actual cause which is mal nutrition. If you have someone like ralph who drinks a gallon or more of alcohol per day almost every day then that alcohol is wrecking your entire body. It kills your brain cells and negatively affects the chemistry of your brain causing moderate to severe neurological damage which as previously stated leads to wet brain syndrome. It also obviously affects your liver and kidneys as they strain to filter, process, and break down the alcohol as well as the usual toxins of the blood which quickly leads to fatty liver, cirrosis, and in short order renal failure which is actually the most lethal part of severe alcoholisim. It is a quick but painful death when the part of your body that keeps your blood from going septic fails and without quick and consistant dialisis it can end you in very short order. The thing that will be most noticable if ralph enters the renal failure stage is he will have jondas (spelling?) Which will cause his skin and the whites of his eyes to turn a sickly yellow and be blood shot, which might already be happening which could be that "grizzled" look of his gunt, but even more likely is the other silent killer of alcoholisim; diabetes.
Fun fact kiddie, with bourban and other grain based alcohols (whiskey and beer mostly) you are pumping your body full of sugar carbs while at the same time overwhelming as stated previously the organs that filter your blood of toxins, including the pancreas which breaks down sugar. This naturally leads to diabetes which can lead to chronic pancreititus and eventual failure of the pancreas causing DKA (diabetic kitose acidosis) or in simpler terms, it makes your blood corrosive. You nailed it on all the major health risks. It's guaranteed he's going to die of alcoholism sooner or later, maybe in the next 5 to 15 years at the rate he's drinking. If he doesn't over-drink one night and get straight alcoholic poisoning and die instantly, he's going to slowly damage his body until it ceases to function. I mean maybe you missed it but a lot of us were talking about how he needed to get into at least some kind of outpatient treatment to taper, maybe consider getting his levels checked to see if he's got a thiamine deficiency. There is a study done on alcoholic Australians that I'm not looking up, but it suggests that the brain cells from severe alcoholism are not killed outright, but disconnected long term. Long term meaning proper brain function returns in a decade or so of quitting drinking. I did pick up on that but didn't see the specific point of death being a real issue on withdrawing once. Yeah pretty much. Im not going to lose any sleep over it for sure. The only way I don't see my description being his eventual fate within the next 3-5 years at current rate is if he falls to one of the other fun ways addicts die, driving while intoxicated (which he has been alledged to do, and eratically at that, or choking on his own vomit, or due to his general health status, widow maker heart attack. Oh yeah thats the really fucked part about hard core addiction of any kind. When you develop a physical dependancy on something your body literally requires it to function and without it your equalibrium is fucked 9 ways from sunday. Its kind of like the matrix, if you die in the simulation your body dies in reality. Ralph will almost certanly require inpatiant detox. They will have to monitor him 24/7 for at least a week or two slowly weening him down.
