From personal experience, there are apps and other places now where you can get prescribed various types of BC (pills, patches, ring, etc.) without going to a doctor's office. Iirc a primary care doctor can prescribe it too. Obviously things like IUDs or implants would need a doctor still. I don't know which one you're referring to in your post.

And lesbians could still require BC too since they can also work to fix or manage hormonal issues.

I don't think they can, but some PCP's can do gyno exams. She could just also be poor and not go to the doctor unless something's seriously wrong (so preventative care like regular checkups just don't happen.) Plus, she probably isn't having a lot of sex if even an exam trips her out like that, so on the rare occasion she actually DOES get laid, she might just use condoms, and all you gotta do for those is bring a quarter to the gas station bathroom of your choice.

This already exists. My wife occasionally watches some Overwatch or Stardew Valley streamer on Twitch who uses one of those avatars (a cheap and creepy looking one) but is clearly a trans person putting little effort into making their voice sound feminine. The juxtaposition between the big titty purple hair avatar and the voice is weird.
