Dexter was a terrible show, but some of the books were pretty good (and one of them was unimaginably terrible), and without a doubt the worst "serial killer" in the series was this one surgeon who would find somebody, tie them down to a medical table with a mirror on the ceiling so they could look at themselves, and then slowly, day by day, remove a part of them while they were still conscious and watched.
He'd start with the feet, then the shins... ect. Move on to the hands, elbows and onward, all while his victim watched this happen to them. And then once he took all their limbs, he'd start with the rest of the parts: their ears, their teeth, their genitals, anus, lips, and tongue, he'd even take their eyelids. He'd take everything until the person was a hollowed out potato, again, while they slowly watched this happen to them. The only thing he'd leave was the eyes, so they would always be aware and could see themselves. Of course their eyes would dry out without eyelids, so he'd have to periodically moisten them with eye drops, otherwise they'd go blind and would no longer be able to see themselves in the mirror.
Then I think he'd toss their bodies out in front of a hospital, where they'd have to spend the rest of their life being taken care of, whether they wanted to be or not. The victims in the story were usually so doped up on drugs from the medical staff at this point that they were basically hollowed out potatoes, with wide darting eyes and horrific grunts and gurgles coming out of their lipless/tongueless mouths.
And then you see a lot of hentai that gets very close to that kind of thing. Makes you wonder if someday a real surgeon will try something like it.
