I'm not sure if anybody already answered this but trans masectomies come in different forms. Small breasts don't get the double incision method, but something else. Keyhole or peri or something like that. I wonder how much of it is the desire to be seen as special due to frustration with not fitting in with and being more frumpy and less attractive/charismatic than other straight girls. This is probably where the insistence that they're "gay" despite being women attracted to men comes from- they don't behave like the other straight women around them but they're still attracted to men, so they start to think their attraction to men is different and somehow "gay".
