The majority of western humans, male or female, live beyond the age of natural female reproductive capacity (mid 40s, on average, extendable with medical intervention, but not recommended. NHS cut off for IVF is 42) and it’s recommended that women hang on to their uteruses as long as possible, so as not to risk things like vaginal prolapse or early onset dementia.
Women are statistically less likely to die in a manner that makes them suitable candidates for organ donation than men are (misadventure mostly, although it’s relatively unusual for
anyone to die in a way that leaves healthy organs behind) so there aren’t going to be many wombs going spare via that avenue.
Women who have their wombs removed before the end of their natural reproductive capacity do so because, almost universally, they have an extremely debilitating long term condition or a life threatening gynaecological cancer, or because they have a high familial/genetic risk of a life threatening gynaecological cancer. These uteruses would not be suitable for transplantation.
This leaves transmen, but most transmen never have ‘bottom surgery’ and if they have a hysterectomy due to cancer risk related to testosterone use, they need to keep their cervix to avoid vaginal prolapse, and transplanted uteruses have to either go into a woman with a cervix of her own (as occurs with some MRKH patients) or be transplanted with a cervix attached.
Thus, the only way to advocate for transwomen to have access to uterus transplants is to also advocate for more women of child bearing age to die untimely deaths in devastating accidents.
