They do do this. It's kind of a pattern on tumblr (and now twitter too I guess) where something with blatantly "problematic" subject matter will gain traction, but the SJW/anti fans will just ignore, denounce, or "fix" those elements and enjoy the rest of the show/game/whatever. Then they also shame or attack people who
do like the "problematic" elements like it's some kind of personal moral failing, even though they're just engaging with stuff the source material provided them with.
This is what SJWs mean when they say "enjoying media critically", by the way.

More likely "okama", from what I know about Grell (I'm not a Black Butler fan but have a little knowledge of it through osmosis). "Otokonoko" is usually for younger characters and specifically crossdressers.
"Okama" can mean a couple of things, along the lines of fag, tranny, drag queen... Basically an umbrella term for effeminate, flamboyant, over the top men. If that was the term used, I can understand why tumblrites latched onto seeing Grell as a trans woman, because "okama"
can mean that, albeit in a sort of derogatory way (and maybe there's some stuff about Grell's character that supports it, aside from flamboyance, again I'm not familiar with the source material). But it is a broad term, so for people to be so rigid about their interpretation is a little annoying.
I'm not an expert but the cultural nuances of homosexuality are also just different in Japan.
