I don't know if he is a full on diabetic yet, he's probably prediabetic. If Ralph was diabetic, I don't think he would be able to give himself a shot of insulin while drunk and the amount of sugar in the whiskey he drinks would probably put him in a diabetic coma and kill him. If he is diabetic/prediabetic, then when he was refering to putting lotion on he could have been referring to yeast infections between his fat flaps so he could be putting on topical anti-fungal creams.
