Being fat is a huge strain on your body, especially on cardiovascular system, taking estrogen makes it even worse (more so in transgender hormone therapy doses.
Just as an aside they constantly misuse hormone replacement therapy - that's something prescribed for menopausual women, what they get is tht - troon hormone therapy). Taking estrogen increases risk of blood clots and which can be even lethal (deep vein thrombosis, pulmonary embolism etc), makes you bloated which is result of estrogen increasing water retention in your body, which also is not very good for your veins. and yes, it does make you fatter.
More on side effects. Another thing worth mentioning that even birth control, which is low dose compared to tht is strongly advised against if you're: 35+, smoking, drinking, sedentary lifestyle, overweigh - doesn't look too good for Jim does it? So, now that I think about it, taking estrogen IS punk in his case - slashing ~15 years and possible early grave for something so incredibly dumb and worthless even should it succeed is kind of punk
