Human behaviour affects how viruses evolve, if it makes you very sick you exhibit sickness behaviour, stay home and see fewer potential hosts. The pressure is for milder symptoms and faster spread. Of course twice as many get it twice as many will be ill, but frankly everyone’s getting it anyway. Japan has done well - as have places that don’t have first world infrastructure like vietnam, and we should be looking at why.
