Well realistically speaking a tactical nuke in a cold war style setting would result in a full blown nuclear war in the near term, like probably within 48 hours. Actually that might still be the case even now.
Depends on whom it was used on. In terms of major powers using them on each other I'd agree, but Germany dropping a few on the USSR wouldn't do much to get anyone else involved.

Deployment of nuclear weapons against non nuclear opponents has been considered several times, but the problem is that it makes everyone else with nukes very very nervous. Like, "Hey, maybe we should see if we can get a counter force strike planned," nervous. The nuclear use threshold is rather high, because once the tac nukes come out anywhere but on the open ocean, gloves are coming off.

In the context of a bunch of commies in the USSR being nuked it definitely wouldn't cause Japan or the USA to declare total war in response. Neither country would commit to total annihilation over communists they don't agree with who would probably subvert their own governments given the chance. The response would be more along the lines of "too bad", though nuclear forces would be put on high alert. We already see this with the German Civil War, where no one else does anything if Germany self-immolates.

Now, if any of the major powers actually used them on each other, that's a different story. "Limited nuclear war" is not a viable option since it would blow out of control at once.

It's hilarious to imagine the actual Rodzaevsky bemoaning the loss of his video game mod to troons. Given he went back to the USSR on a false promise of freedom and was shot, he wasn't the most astute character. He would, however, have fitted in very well with the weebs on the subreddit.
