As far as pain goes they really are not sure. There are definitely signs that the body registers pain from the brain stem, how much pain is perceived by the infant is still unclear. I can tell you that they show signs of pain, whenever doing a painful procedure you can see the heart rate and blood pressure increase, or do the opposite and the heart rate plummets into severe bradycardia. It sucks to be the one inserting IV's and blood draws knowing it's both pointless, and you sincerely hope that they don't process pain.
I read about Tinsley. I remember there being discussions as to parts of her body dying and decaying, many people day did not think it was possible. It absolutely is possible! When the body begins to shut down, blood circulation to the extremities slows and even stops. Reading that fingers, toes, nose are rotting was probably going on . I once took care of in infant in septic shock. He lost circulation to his legs, and the tip of his nose. After several doses of antibiotics, and vasopressors some circulation came back, but the tip of his nose was narcotic and had to be removed. Even worse his left leg from below mid thigh had died, and was mummified, until he was strong enough for it to be amputated, my fear was accidentally hitting it against the radiant warmer and it breaking off. It was horrific and he actually lived.
The sad thing about amniotic banding is they usually have a normal brain as long as the head and neck were not constricted.
So many crazy parents, I don't know where to begin. The worst are those that insist everything be done, but rarely bother to visit. When they do show up they demand food trays, hotel vouchers, and cab vouchers. They preach to you thier baby is going to be the miracle, and they totally ignore you when you tell them that. you just got their pain under control and they are resting. Instead of waiting awhile, they insist you wake the baby up and handle them because they want to hold them when convenient for them.
Many of those who let their baby suffer for months, when the time comes and the baby is actually dying, they can't bother to come and hold them. I spent many hours holding a dying infant because they are human beings and deserve to be held rather than die in a bed.
The drug babies are the hardest, not the most interesting as defects, but otherwise perfectly healthy babies n o much pain from withdrawal, in spite of regular doses of morphine. They cry for hours on end. After 12 hours I couldn't take it anymore. Yet those parents will visit so high on drugs and somehow despite losing any other children, they are still given a chance. It is shaken baby syndrome waiting to happen if they are not dumped off on relatives first
I sperged out about this re: the Hooligans, but essentially the midbrain is required for sorting the signals from the brainstem. Without the midbrain, pain isn't 'perceived' as anything. It occurs, it is reacted to, but there is no awareness.

In cases of severe malformation, I'd argue that the sensation of pain is altered, and may even become mixed with other sensations (like itch). They'd be aware of it as 'something' that may or may not register as unpleasant.

For potatoes, I've heard people say oh, they couldn't move their hand from a hot stove. But these individuals rated low on the Glascow scale, so I actually think the individual didn't even perceive the pain as pain.
 
I've always heard that the hardest thing about being an OB nurse is not the dead or severely ill children, but all the kids that they really don't want to send home from the hospital, and that doesn't necessarily mean that they're teenagers, low-income, etc. There are highly educated, wealthy couples who are probably even more dysfunctional.

I just saw an NPR story via Facebook about what COVID is doing to families (AGAIN) and the women who chose to reproduce with man-babies have come out of the woodwork in droves.
 
Yeah while Gwen was nowhere near as bad as Luna's mom is with medical neglect I still feel like she still did some pretty shitty things like never getting Lola a feeding tube, cuddling her when she had shingles (and was surprised when Lola got chicken pox), dragging them everywhere during cold and flu season, and denying them proper pain meds. As fucked up as the Hartleys were they at least deserved to be more comfortable if they were going to be kept alive.

Has Gwen really done much other than post some old pics after her spuds died? I'd be highly surprised if she hasn't slipped into Sydney and Charlie's moms DMs, or the DMs of any other mom with fucked up kids like that.

I agree, she's not as bad but she still terribly abused them. I always hoped they couldn't feel pain because if they could it doesn't bear thinking about. It's too horrific.

It was a blessing when each of them died, which I hate to say about any child but it was absolutely true. It was the only mercy they were ever granted by the universe.

I'm 100% sure Gwen contacts parents, somehow. Definitely Sydney and Charlie's, they're too similar to the girls for her to resist. I always wondered how she would vicariously live after they died; through Cal or through other parents. It wasn't him.
 
It depends on the severity of the condition and whether it's associated with other genetic disorders. An umbilical hernia, which a huge percentage of babies have, is technically a mild case of gastroschisis. I first heard of the condition, by technical name, when the place I worked at got a letter from a former co-worker who had a baby with a pretty severe case, although this baby was able to have a normal life after several surgeries. IIRC, they were able to place her exposed bowel back where it belonged, and did a couple other procedures to strengthen her abdominal wall.

The one person I have known personally who was born with amniotic band syndrome is in his 40s, and he has one arm that's normal and the other one has an elbow joint with a thing attached to it that looks like a finger. Otherwise, he's completely mentally and physically normal.
Indeed, there is another condition called omphalocele, in which the abdominal organs protrude through the belly button. It differs from gastroschisis in that the displaced organs are contained within a thin, membranous sac.

A previous poster is also correct in saying that gastroschisis is believed to lie on the less severe end of a spectrum that includes limb-body wall complex, which is another of those pesky conditions fundies insist is totes not "incompatible with life". I believe it's what the inside out baby posted about earlier in this thread had. It isn't a very well understood entity.

Amniotic banding syndrome is one of those conditions that can range in outcome from "catastrophic" to "nuisance". Just depends on what parts fall off.

Yeah nah, the A&H crowd are a bunch of incels that don't realize abortion happens regardless of legality and some conditions make it necessary (like ectopic pregnancy). One just recently called women and girls dying in illegal abortions "cosmic justice" so yeah, it's clear they don't actually give a fuck about life, just embryos.

Whining that that the SC abortion ban doesn't protect "children" like her daughter because it makes an exception for fetal anomaly. Pretty sure there's no way to "protect" a fetus with limb body wall complex, the best thing you can do is abort it before it develops the capacity to suffer. Something Sarah refused to do, so hers spent almost 2 hours in agony while it slowly died from it's organs being on the outside.

Access to late term abortion is still pretty iffy, as it is in most states. Unless you live in one of the few states that allows it, you're on the hook not just for the procedure, but also for airfare/hotel/car rental/etc. Getting a late term abortion to spare your wanted kid from suffering from some horrible condition requires having a fair bit of financial privilege, I can only imagine people who can't afford it have no choice but to gestate and birth doomed fucked up pregnancies or get induced early.

It's not related to tard babies per se, but my state is also an abstinence only ed state. I think any other Kiwis from abstinence only states can back me up when I say it just increases shit like teen pregnancy when kids aren't even being taught how to use a condom.

Personally I think all states should have comprehensive sex ed and access to late term abortion, because forcing poor women to give birth to doomed babies who will just suffer because they can't afford to fly across the country is unbelievably cruel. Inducing early sounds like it's a decent work around, but I'd imagine if a fatal diagnosis doesn't happen until fairly late it's probably not as effective at preventing suffering.
 
Abortion access in Canada is an issue cause of how spread out stuff is and rural population. PEI probably still doesn't have its own.

There's ONE case of body-wall complex surviving till 5 years old with cutting edge science that tried to make a flesh bag to stick the organs in. Imagine the life that kid had to live :(

Yes, with infants and children they usually suffer more brain damage every time they code, CPR is not as effective at circulating blood and oxygen so often there is more brain injury which is why her not being a DNR is horrific. When an infant does not move normally as they grow, the muscles and bones don't develop properly either. They often suffer from neonatal rickets and get many bone fractures. And CPR causes broken ribs in robust healthy people. Broken ribs hurt like hell!
There are those people who honestly believe they will get a miracle, who despite being blind to the suffering, do care and fist often, and call to check in. Then you have the others who rarely visit, demand meal trays, cab vouchers, etc. They sit at the bedside eyes glued to their phone, and unless they are in the mood for a photo shoot, they leave without ever touching their kid, It's all about themselves.

There is so much information regarding your questions, I would have to answer each one separately as to each infant. Will do my best , please forgive me for any bad spelling, grammar, etc. typing long stuff on phone difficult, especially after a 12 hour night shift.
Regarding Tinslee: I have taken care of infants like her, but once they reach 9 months to a year old they are transferred from the neonatal ICU to the pediatric ICU.
Infants like her are truly living in a world of pain and hell! Mechanical ventilation while breathing for them, also does damage to the lungs due to the pressure required to inflate the lungs, and the more damage and scarring, the higher the pressure is needed so it's a terrible cycle. Being on a ventilator is horrible: the vent gives a set amount of breath at set pressures per minute, if you have a functional brainstem your body naturally wants to breath on it's own, but cannot because when it automatically tries to breath in or out, the vent is chugging along and so you may be trying to exhale while the vent is forcing a breath in. Breathing through a breathing tube and vent tubing feels like what you would experience if you took a long straw and tried to breath through it, this all makes you feel as though you are suffocating.
Since she is on artificial ventilation and tube feedings, she would get blood work drawn daily and more frequently. She would likely have central lines or PICC lines, but those don't last forever, the clot or have problems, requiring multiple IV' s until a new central line is placed. Often if a blood transfusion is needed or a medication that can not be mixed with others, again more IV sticks.
Due to not being able to move normally as she grows her muscles and bones would not develop properly so the bones are weak ( called neonatal rickets). Due to weak bones and chest muscles, CPR on her would be horrifying likely resulting in several rib, sternum, and vertebra fractures. CPR mechanically circulates the blood until the heart restarts but is poor circulation compared to the heart so after you get them back there can be significant damage to the brain and other vital organs.
I see why the medical staff is traumatized caring for her. Skin issues are common with immobile patients as well, babies like this are hurting and feel like they are suffocating just laying there, changing the diaper, positioning, bathing and doing procedures on them pushes the pain to a higher level. I have had infant's literally code as I gently lifted their bottom enough to slide out a wet diaper and slide in a clean one

As for the Harleys again that is horrific. Those girls lived a lot longer than they should have. In my opinion the mom was selfish. Most people would rather have quality over quantity of life. I cannot see anywhere any proof that at any time those girls ever felt pleasure or comfort. Thier entire lives. At the least they felt nothing, to at the worst feeling suffering and pain, and all just to be able to send smug letters to a doctor every year?
Besides what those girls may have gone through, those parents anger me for other reasons. I also did some infant/ pediatric home health care. The cost of keeping children like that alive is in the millions per child per year. Even if they started off with insurance all plans have a maximum pay out. We the taxpayers support them via social security disability, medicaid and many other state and federal programs.
We pay for home nursing care, respiratory therapy, speech therapy, play therapy, physical therapy, occupational therapy. Multiple doctors are required GI, orthopedic, neuro, hemotologist, nutritioniste is the ewuiand surgeons as those feeding tubes need to be revised. That's just the people involved, then there are the braces, casting, special beds, bathing chair/ beds, wheelchairs and other mobility equipment. Feeding pumps, oxygen equipment, tube feeding formulas, handicapped vans for transportation safely. The increasingly larger diapers, blue underpads and so much more.
What passed me off the most is that most of these parent don't work, or only one does, yet they live in very nice homes, homes that as a full time nurse I couldn't even dream of. The cost of most of the medical stuff is covered, yet they get income from multiple places individual donors, grants, fundraisers, etc. I mean the Hartleys even managed a free hot tub. From the vids they appeared to have a nice home, they must have had a handicap ped van or type of transportation. I bet they also had a couple of nice cars aside.
It sucks as to walk in to a house nicer than you could afford, then you do all the shit work such as bathing, changing, feeding and they often ask the nurses to do housework and laundry as well, while the parents sit on their ass. The times you see Gwen actually doing bathing or diapers, etc, usually those kind of people only do when the camera is on.

With the catastrophic anomalies discussed in this thread, any semblance of "a life" ended the instant meiosis went awry, the amniotic bands wrapped around a vital structure, the blood supply to the fetal brain was temporarily disrupted at a crucial moment, the neural tube failed to close. Abortion is just finishing the process that nature instituted. The anti-abortion crowd love to talk about life beginning at conception, but in many of these cases, death begins then, too.

This is especially true with the drug withdrawal babies. They have an almost constant grating cry, after a 12 hour shift taking care of one, I can't stand it, and I got to go home. Even if the mom' s do everything right, get off drugs, and do everything possible to get their baby, I feel sick knowing that. Mother is still dealing with her own withdrawal and issues that led her to addiction, imagining them having to hear that cry 24/7 , I doubt most could cope. I always hope there is some family member that will help, if not I wonder how long till it's a shaken baby in the PICU

The one otherwise-healthy narcotic withdrawal baby I remember was one where they were prepared for it, because the mother used opiates for a legitimate pain condition, at a high enough dose that the baby was born physically addicted. It was more dangerous for her not to take the meds. That baby was probably weaned off and went on to lead a healthy life.

Otherwise, yeah, those babies often have multiple other problems and go straight from the hospital into foster care. :(

An arterial line can be inserted even in a premature infant. In fact, in neonates, there's an ideal site - the umbilical artery. It's fiddly work, for sure, but it is definitely possible and likely preferably to the more invasive and painful alternative of repeated arterial sticks.

Well, I'll be dipped in shit. That makes perfect sense. Didn't think about the umbilical artery. I ain't seen anything more than a temporary vented peds patient with repeated sticks, and only seen an arterial line on an older child.
 
Dumb question: Would she have an arterial line at her age or do they stick her for an ABG?

I would have to guess that she has a Central IV line or PICC line.( maybe both, maybe multiple) They can run a venous blood gas which is not as accurate but sufficient enough, as long as they are no sudden major changes in blood work, venous blood gas results or drastic vent changes.
Then they would probably do an as needed radial artery stick. Those are painfully as hell as you can't feel the artery like a vein, you go by the area you feel the pulse and anatomy, and dig around until you hit it. The older you are, the bigger the vessels and it is to feel and hit an artery just like a vein. I am only going by what I have seen on infant's in similar ( probably not a severe because most are not kept alive that long, they usually die from an infection as thier bodies have no strength for an immune response, even a minor infection quickly turns into septic shock).

An umbilical artery and venous line are not useable for a prolonged period of time. Weeks at most. They are put in under sterile conditions, as the infant grows and as the cord dries up, shrinks, and retracts the line is not able to stay inserted at a depth enough to be in the correct place inside. They cannot be pushed in further as that is not able to be done using sterile technique. They are easily dislodged, and prone to infection and clotting. With a micro preemie the standard is to put in an umbilical and venous umbilical line to use until a PICC line can be inserted, then they are removed.
Umbilical lines are amazing, but temporary. With a baby as old as Tinslee, there is no way she has an umbilical line.

Oh, right, shit. I didn't realize we were talking about poor Tinslee specifically. I shouldn't be posting and "working" simultaneously!

Also, I think my brain just kind of wants to elide Tinslee's existence altogether. It's too horrifying to contemplate.
