Her ankles and feet are quite swollen. We've seen that before, so it's probably not shopped. Her circulation and lymphatic system in her lower legs is fucked.
It reminds me of reports from the remains of cannibal victims. The Donner party, Chilean soccer plane crash, various lifeboat-stranded doomed sailors...the larger bones of the eaten people all had distinctive scraping marks from knives cutting off the muscle.

Marks from when she used to do... whatever she does... to her whole legs. There are pictures earlier in the thread. It's easy to forget she did that considering the current state of her shins.
Yes, it looks odd. But there have been several previous pics from different angles, including closeups, that show swollen ankles and feet.
