Sounds like a botched surgery, especially since metas are supposed to be very simple and have relatively few complications.

It's not. It's just the result of using the pump. The tissue reverts back to it's regular size (in this case a way tinier roid clit) after pumping (but I don't know how long it takes).

This seems to be a hallmark of the worst of these butchers. Instead of specializing in one thing, they claim to do everything, and invariably, just churn out mutilations factory style. This person has a real Dr. Rumer feel.

I do think there are troons who lie about getting the surgery — because it’s a fetish. That’s what makes it so dangerous. If you see a post by a guy about getting railed by his werewolf boyfriend, you know that’s fake. If you see a post about how great post SRS sex is, a stupid person might think it’s real.

Labiaplasties are things real women get, MtFs can't get them because they don't and never will have labia. Though I guess saggyemptyballsackskinplasty doesn't really have the same ring to it.

apparently, it's a plastic surgery to reduce the size and/or change the shape of protruding labia to conform to societal standards. they're not really for medical reasons whatsoever and are rooted in stereotypes of what a vulva "should" look like. it can make women and intersex people feel more comfortable with their genitalia, whether the issue is genetic or due to age.
as you can see in these images, the woman already has labia to perform labiaplasty on. interestingly, crafting fake "labia" for trannies is also considered labiaplasty. I'm not surprised trannies feel like they should be included in this surgical procedure meant specifically for people with female-presenting outer genitals (I'm including intersex people here)

this image is captioned 1 year after vaginoplasty, 3 months after labiaplasty. what i think happened is the attempted "labia minora" fused with the "labia majora". There's no image of the vaginoplasty, but we can assume the labiaplasty didn't do much here.
