he was probably also prescribed metformin though, which is pretty standard for type 2 treatment. Insulin, as mentioned at the begining of this arc, is not exactly normal for every type 2. Lou acts like he's the average type 2 patient, when insulin is only prescribed when drugs like metformin alone aren't enough.

Experimental oral insulin was in the news recently, so that's another "yeah we'll totally have it in five years oops wait NM." He might have wistfully reblogged that if he were on insulin, though. I remain surprised nobody's taken Lou to task on diabetes, specifically his diabetes, being a disability.

I also think he probably discharged on orals: the main reason he wouldn't get metformin as a first-line would be if there were concern for his renal function (and that could just be acute, from the antibiotics for osteomyelitis. Since he has a stepdad who's on dialysis, anything about kidney function would put him into an area of (relative) expertise and would certainly be folded into his begging routine.

Mistakes and optimism can happen, but prescribers do take mental ability into account when deciding diabetes regimen, and they had all the time waiting for sensitivities on the foothole to work on the rest of his routine. Lou's performance with his diabetes educator is going to be in his chart notes now; this is not a guy who can be trusted to reliably assess carbohydrates and base rapid-acting insulin accordingly. The most I could see him on is a dose of Lantus/Levemir/Tresiba, but again: there would be specific needle-whining, and insulin-begging. Metformin, glipizide and glimepiride are on the $4 list.

And it'd give him specificity to his next commission; he needs a pumaliontiger with injection marks and an alert bracelet because of his disability.

Corust isn't saying anything about Sparx that Sparx hasn't made publicly available about how he views transitioning as a kink. He literally said it himself about himself.
