Something I’ve wondered about, do Munchies experience *physical* pleasure from medical attention? Like some junkies go for opiates or uppers, but these people just need a hit of Doctors & Nurses Fuss Over Me? What’s the reward the brain is giving them? Is it a craving for gentle and/or competent physical touch? Presumably there have always been illness fakers, so what role does the broad institution of “medical science” have to do with this? How did it manifest, say, 500 years ago? Did they just pretend whatever old shit & get shipped off to a convent so put-upon nuns could sponge-bathe them until they all died of typhoid?

The reason most psych reports often begin with a description of the person's behaviour and appearance at the time of meeting is that it is a part of assessing mental state (are they dressed appropriately, do they have non verbal responses that match up with the situation?) not picking out random crap like being 'pleasant' or left handed with no justification why.

Just noticed she lists 'Petit Mal seizures' that way of refering to absence seizures hasn't been used for years and she's only 27, I wonder when she was 'diagnosed' and whether she's seen a neurologist recently. (of course she won't have).

Update on Emerald / Ticsandroses:
She deleted the fake diagnostic report video and has disabled commenting on all her TikTok videos.
She uploaded a video that's just text announcing she is taking an "indefinite hiatus."

Her hiatus lasted a couple hours before she came back to leave angry comments blaming the haters.

People were able to figure out the name of the doctor she used for the fake report because of how bad her attempt to redact it was. I've heard she misspelled the doctor's name but I can't make it out enough to confirm that. Angry Redditors are emailing the doctor and the hospital about this.
She has purged her ticsandroses Instagram.

I took the time to look up all the links and people and, curiously enough, none of her supposed family members mention anything about Huntingtons disease. At least some awareness posts should show up, considering that Rose herself said there's a "family history" if I remember well.

When you see diagnoses written in a numbered list like that by medical professionals it's usually ranked from most to least significant in that doctor's pov (if you're a psychiatrist you'll rank mental illnesses higher than physical illnesses; vice versa for those who are being seen for physical complaints). That's what hit me before anything else on here because there's no discernible order to the listed diagnoses. There is no way a doctor listed fibromyalgia as number 2 because it's not a treatment priority for any doctor. I'm not sure what sort of doctor she's pretending to have seen but if it was a neurologist then they would have grouped all those neuro diagnoses together for sure, so would a psychiatrist. See here for a derm template/example.

People have already talked about the diagnoses being out of date and other mistakes so I'll try not to drone on but to add to the list: anemia written on its own is about as useless as she is. This is a huge clue because there are so many types of anemia that have different pathologies (B12 deficiency anemia can be seen with neurological signs so would be pretty important to a neurologist). Migraine would always be written as migraines with/without aura. Finally I zoomed in the video to see what else the letter said and the history of presenting complaint is devoid of medical terminology that makes it actually useful to another professional reading it; it's written like a biography when a competent neurologist could take a paragraph she's written and condense it into "describes childhood verbal and motor tics". She must have had a real ego-stroking session writing this one because you'd have to be pathologically self-absorbed to think her detailed disability fantasies would ever be that relevant.

As a side note, it's very common where I'm from to begin new patient letters with "I had the pleasure of meeting Ms. X" "I met this delightful X year old gentleman" so much so that if that has been omitted then the recipient has to play a guessing game of whether the patient was annoying and pushy or if the sender is the unpleasant one. It's old-fashioned and becoming less commonplace. "Very pleasant lady" therefore didn't throw me off (I agree that handedness was out of place in this part of the letter even if relevant to EEG) but the high formality then degrades into "as a kid" lol. If the Americans here are saying that pleasantries/formality are unusual for US doctors it's making me think perhaps she found a medical letter template off Google that had a non-American source.

Agree the very pleasant lady part is common in Europe but I've seen letters which specifically are only about issues in hands and they don't open like this. It's weird.

I have no doubt she faked this, but it’s a decent enough fake. This looks like an internal document which would be a part of her chart/EMR and would not be provided to the patient, unless specifically requested.

It is common in the US to start out with “Pleasant 23-year-old female presents with xxx” for example. Of note, the diagnoses would be provided by the patient, probably exactly as she said them. No one goes into assessing a patient thinking that it may be a munchie, they just assume the patient is telling the truth (which is a good thing).

As far as the HPI, doctors often use the patient’s own words, although the sentence usually starts with “Patient claims”.

I would really like to see the second or third page of this document, because that’s where you’ll see the test results, the doctor’s prognosis, assessment and plan. That’s where you can figure out if the doctor thinks this is bullshit or not (even if it’s subtle).

As previously said, describing a patient as pleasant is very common practice. Though, I can try to elaborate a bit more on why. This practice prepares doctors and assistants for dealing with patient care. It’s always relieving to review office notes pre-visit and see the patient coming in will not be ... well.. unpleasant. It also serves as a warning for referral coordinators and specialists. When receiving a referral and the notes are straight to the point or even mention prior “incidents”, you know the patient is likely to be unruly or an asshat. The staff can mentally prepare for these patients beforehand as seeing many patients of an unpleasant nature can be very taxing.

As for EMRs, the diagnosis codes are not grouped by ”most concerning issues” or ranked based on severity as someone said previously. Most EMRs automatically group by issues addressed during this visit (for billing purposes), and then diagnoses charted from previous visits that aren’t being addressed today. They can be ranked in these two groups alphabetically or by the order they were entered. This can be as random as going down patient paperwork and entering in diagnosis based on a predefined checklist.

Finally, the reason for informally-written diagnoses such as “Tourette’s” is because those diagnoses are entered by medical assistants, front desk staff, or nurses, and not providers. When a diagnosis is entered in unprofessionally, it’s likely entered by a method called “Free Text” which is when the EMR isn’t aligning the diagnosis with a code, but rather an assistant put it in manually. These are also not official diagnoses, but are considered self-reported and are treated as such.
