I think the key difference is the simple fact that Clay pushes this false image of himself instead of adopting the persona while also being true about himself. It's extra weird because his fans overall would probably be very ok with him being obese and he could likely capitalize off of it in a few different ways.

The issue (among many) is that he got his high school athlete brother to be his body double, and then made social media posts using his brother, and then pretended that his brother was himself.

If you take a picture of your brother's arm petting your cat and then say it's a picture of you petting your cat (which Dream did) , it's a lot different than performing as some homoertoic anime character, it's trying to claim that the PERSON was someone different than they are, not the avatar.
