Yeah more realistically he'd be a pudgier/skinnyfat looking version of his brother with a not so prominent nose assuming the fat isn't hiding most of it

Deep inside he knows he's disgusting, he knows that he's ugly, and the fact that his ex just dated him for his clout reminds him of it everyday. Dream isn't insane yet--there's still a rational mind in there that is disgusted about who he is and scared about the mythos and hype that he built around himself as a young slim uwu twink boy biting him in the ass.

He is immensely insecure because finding financial success in a children's block game while being an unhealthy pathetic drop-out is still shameful compared to his more successful siblings having athletic pursuits or medical careers.
