They are very different things, but medical insanity can come into play with sentencing. If he has a competent lawyer, IF, then they could realize there is no way they are going to get out of this and that their client is just going to keep digging till they hit China because you can't shut him up. You can use his medical insanity to at least get him put somewhere that he will not be in danger form other inmates. The dude is seriously fucked in the head and has needed mental help for probably his whole damn life. I'm not sure that he'd make it in general pop. They could do a plea deal that gets him incarcerated in a way that will punish him but also keep him safe enough so that he survives his sentence.
