back then, even a minor wound could be fatal. bacteria with no antibiotics. so the procedures were done under scrutiny, with aftercare, extreme amount of follow up, by a very select few surgeons who repeated the surgery exactly, again and again until familiar with it.

and still, plenty of them died. for every successful eunuch there were ten dead ones.

these days it's all about the lack of a standard process and approach, no follow up or aftercare, lack of compliance and cleanliness by the patient, and attempts at strange, unresearched operations. coupled with a firm lid on discussion of results and problems within the community, it's a worse scene now than then.
