Personality disorders come in various shapes and sizes but all relate to a disturbance in, well, the personality, but in other words the sense of a consistency of "selfhood" and identity is diminished and behaviorally people take one of a number of different routes of dealing with this whether mostly directed inwards (e.g. schizoid, avoidant) or outwards (antisocial) or some mix (borderline) but they all have in common the fact that the capital-S "Self" is disturbed and diminished. And this is to a varying degree. Perhaps, even, the Self can be so diminished that it is extinguished, and here we have the true sociopath, or, in religious circles, objective metaphysical evil and even possession. To be clear I'm not saying either of these things are the case for Bella, talking about total emptiness or demons is a bit over the top since none of us have actually met her, but it's clear that she is extremely disturbed and the root of this is a lack of strong sense of self whether that comes from childhood abuse or is inborn or anything else, we don't know.

I think it's part of the whole cluster B thing- not in a 4d chess way, I don't think she is smart enough to purposefully play such a long con with everyone in her personal life, but I think that she code switches more than anyone with a healthy psyche does as a result of her diminished sense of self. If you don't have a strong grasp of who you are because your morals and personality change based on who you're talking to and what you want out of them, you're going to have a lot of masks to cycle through.
