The "segregation" phrasing is definitely to rhetorically signal at his audience (who are probably mostly American, liberal white ones at that) who are familiar with the history of racial segregation in the United States— as if a horrific system of racial apartheid and putting someone on a waitlist for a non-essential surgery (for a niche procedure that a very small fraction of the general public has) are at all comparable. It's like saying a hospital is "segregated" for having one wing for pediatrics and another for oncology. What a dishonest charlatan.

Short telephoto like 85mm is most basic bitch standard when shooting portrait photography, so it is very safe to assume that this shot was taken on lens like that. I doubt that photographer took 200, not to mention 600mm just to make him look more flattering.

However, it is common misconception that focal length changes face features. It doesn't. Distance from target does. If you shoot a with with portrait with portrait lense with one hand and your phone in other, facial features on both photos would be the same. However, your phone has much wider field of view, so you would need to crop in quite a lot.

I don't think that professional photoshoot was heavily edited. Maybe some work around jaw. I think you underestimate how much flattering lighting, correct distance to your target and good angle can change photo.
