How do you hit that many people in a tight space, generally speaking you cant because meat stops fragments and in a tights space you have like 20 people tops in a direct line to you.
Keep in mind the vest has to be light enough not to be detected on the way into the airport.
So yeah, the 120 casualties the US is claiming don't pass the smell test.

Your interpretation of casualty is wrong. Casualty means anything that could take a person out of active military duty, this ranges from getting blown up and shot dead, to wounded in the aforementioned way, to simple things like concussions, disease, or combat fatigue. Disease used to be more likely to kill you prior to modern medicine than getting shot or blown up, this is why the term casualty is so broad, militaries needed to be able to tally up more exact figures than just KIA. When the government is saying there was 120 casualties they mean people concussed by the explosion, mutilated by the blast or shrapnel, burns, people who have inner ear damage and can't walk properly or hear things, and those who have just outright died. The number seems reasonable for a car bomb in such a packed area as the airport and it was something I had been expecting.

I disagree, the problem is the number of bodies shielding others from the blast. a human body is pretty fucking good at stopping shrapnel.
