They have to know they have a major problem on their hands. Just imagine though, he comes back on twitter as reformed Christian Spoony or something. Quoting the Bible and how it relates to his downfall.

I think Christianity and religion in general is fundamentally incompatible with Noah's personality. He needs to nitpick, question, complain, and poke holes in everything. I don't think he's capable of being told something that he's supposed to literally take on faith. It's not cause he's some truth seeker searching for some greater philosophical understanding. It's because he enjoys complaining and tearing things down, and also because if he can denigrate any new belief system to the point he can safely convince himself it's bullshit, he'll feel he never has to change.

I was mainly saying he would be the type to fake it until he makes it. Doesn't believe, but could do anything to keep the fire off.

Just a possibility if he ever comes back, it won't be sincere either way.

The guy can't even wipe his own ass anymore, let alone do something he actively doesn't wanna do.

I am really happy for Oreo. I think it'll do wonders for her to be around normal people. I know she's getting up there in years, and I think she'll live longer with the Antwiler parents around than she would just with Noah. I feel awful for that dog having to spend so long in a dark filthy house devoid of food with an out of shape version of the Frankenstein monster's depiction from the novel.

I honestly think his folks mandated no internet when they took him in. Both as a way to protect themselves from trolls and to try and get Spoony in a more healthy place. A lot of his depression stems from what is basically social media/internet addiction. (Obviously not all, but it's a major contributing factor.)

In addition, I hope they mandate him getting and holding a job. (Even something as low as burgerflipper.) Also, I'd personally force him to do something social on weekends and maintain it. (Like playing, not DM'ing a D&D game and mandating he can't alienate himself from it if he is under their roof.) Try and get him out of the social interaction death spiral. Make it something quasi enjoyable based on his hobbies to try and hook it.

They have a long road with how toxic his personality is, but small steps.

Since it's been over a month you GOTTA be wondering, what is he actually DOING? Twitter was his life and pity was his hometown, and he's had none of that for 6 weeks (ish)
Considering he passes out thinking about what game to play, what he's gonna do during the day now? you cannot just sleep all day everyday and never leave the house
Work? hobbies? chores?

I'm pretty confident that they have him doing chores around the house and looking for either a job or a trade to learn. He's probably seeing a new therapist and being forced to choose if he wants to take medicine or actually follow the behavioral therapy. I highly doubt his parents are letting their forty year old son play games all day, and if they are, he's probably giving them all his Patreon money for food and rent if he's just going to treat the place like an appartment. I'm just going by what the average reasonable parent would do for someone like Noah moving back home at forty.
