There are many hospitals that serve as "training facilities" for these such things. Usually they tend to reel in people like Phil with promises of cheap or free procedures that are technically paid for by the taxpayers. I'm pretty certain paperwork is signed that absolves them of any responsibility if anything were to go wrong.
Maybe the west coast is doing the country a favor by sterilizing all these troons.
