It's a low bar; as long as the defendant has a basic idea of what the court proceedings are then they're fit for trial. Still people will feign insanity thinking it will get them out of trouble. In reality the trial can be postponed indefinitely if the charges are severe enough with the perp being locked up in a nuthouse until deemed fit for trial. Even a successful insanity plea simply changes a prison sentence to a commitment to a state hospital (e.g John Hinkley jr.)
