My god she was so thin in college. Obviously she wouldn't have much muscle mass, but she looks cachectic. I assume her ability to eat/swallow was better then than now, but maybe not? (as she's on the "twerk juice").
This is one of the things that really interests me about her: she'll talk about anything under the sun, including her inability to wipe herself and her child's paternity tests, but she never wants to talk about the period of her life when she was extremely thin. For most people, I'd just understand that it's often a sensitive topic, but Alex loves talking about herself-- anything
at all about herself. The two things she seldom discusses are her weight and her own home situation (with her parents). I've wondered if there was more going on with her thinness than the obvious physical issues, but she won't say.
She's also gained a lot of weight recently. "Gained weight" is a very relative thing, and I'm sure it's good for her to be able to hold onto any weight that she can. Her belly is what's known as "SMA belly" and is disease-related rather than weight-related. Still, though, I think that her recent weight gain is puffiness-- fluid-- rather than non-water weight, plus whatever calories she's getting from drinking alcohol.

Your weight doesn’t make you any less or more worthy.
I’ve posted a few pictures from my childhood where there’s a noticeable difference in my weight. I’ve kindly asked people not to comment on my weight in those posts bc those posts had nothing to do with my weight and yet that’s the first thing people noticed and felt the need to point out and comment on. That’s something I think we really need to step back and examine: unsolicited weight gain / loss comments. Everyone was commenting how much “better” & “healthier,” I looked.
Your weight doesn’t make you look “better.” Yes, there’s a huge difference in my weight — but that’s between myself and my body. And as far as healthy goes... healthy is a myth. What’s healthy for one person might be completely different for another. I have what’s technically considered a fatal, progressive neuromuscular disease — yet I’m completely “healthy” for me. Everyone with my disease is different and it effects them different & everyone in general is different.
I’m very happy I gained weight not because I look better but because if I get sick — it won’t be as dangerous as when I was 42 lbs. I do feel better. And that’s all that matters.

It's interesting that she looks like she's holding a little more weight, when all of her other physical signs are deteriorating. Most people would probably get that checked out, particularly if their overall health was like hers.
