The cafe incident (when he was wearing the school girl fetish costume and scarf in public) was only 8 months ago and he's made no change since then. Alex also cannot put a drop of effort into anything except video games, so I severely doubt he was able to commit to even one lesson in voice feminization.

Alex, you don't seem to understand how people can see you as male even with female clothes and make up, but trust me, people can easily clock you.
Why? Because you have receding hairline, a fat nose, a male brow ridge and a stubble, all those things are what women aren't born with (except the fat nose), and I only mention your face but you also have an adam apple with a deep voice, you also have man mannerism (the way you talk and sound is male and I can see this through my screen, I'm sure you walk, act, and move like a man and not a woman IRL).
Passing as female is about many things you're incapable to understand because you have no idea how women act, that's why you don't pass, even with blur and ugly filters, people still see your maleness.
People outside don't misgender you because they're polite, they see you in a dress so they assume you must be trans and they gender you as female, but they don't see you as one.
Why? Eh, I started losing my hair at 24. It's not a big deal. The problem here is that Alex is so god damn superficial. Most young men that lose their hair cry for a night or two, then shave it off and go for a "bad guy" type of look. Alex? There is no way he can pull off a female look with that. It's not even a mild case, those "horns" are going to the middle of his head, at least based on the hairstyle he has.
Not entirely sure how HRT affects hair loss though. My amateur guess says it gets worse because of how you fuck with your hormones, but I could be wrong.

High estrogen actually helps prevent/reverse hair loss; that's why most women don't start having thinner hair until after menopause when estrogen levels drop. NCBI has a case study out where 6 mo HRT in an MtF helped restore SOME hair growth, but the pics didn't seem drastic, and it was a single case study.
All in all, my best guess is that it might help a little bit, depending on what exactly he's taking, but I would doubt any major improvements once its already this far in.

with any HRT you are told to also take vitamins with it because it can effect your body. a multivitamin and some serious Calcium-d-glucarate is best in his case. he could use some biotin it could help his hair some. oh and not smoking and eating pizza hut, hot wings with mountains of useless toppings + gas station sushi like religion helps helps but u kno.
