Two options. 1) They're getting their dicks wet in her petri dish; 2) she's bringing in enough money for them (via ad views, tips and subscriptions) that they've decided it's worth the PR hit to bend the rules for her so long as she doesn't actually fuck her dog or kill her cat on camera.
Neither would surprise me.
Just take a moment and think about the kind of technological innovation (and raw hardware resources) it takes to automatically examine every fucking thumbnail assignment attempt that comes into their platform every minute (three auto-generated per video, plus an optional user-submitted thumbnail), compare it to an ever-growing list of "no-no" images, and programmatically choose and enforce an "acceptable" substitute thumbnail from the offending video that avoids displaying the "no-no" content while still representing the video, and do it all during ingestion instead of after-the-fact. And be able to add to the "no-no" list on the fly with little (or no) delay between submitting the new forbidden image and it being included in the "no-no" scan.
Then imagine being in control of such a system and deciding its best possible use is to prevent specific e-thots' faces from being used as thumbnails. Now just think about what humanity has potentially lost because all that ingenuity and those computing resources were wasted on this horseshit instead of something beneficial to mankind.
