LOL. I'm stoked that I actually called this: a doctor's note excusing her from work! Not that I believe this shit. The date is typoed, the test name is wrong, and most doctors offices - when dealing with shit like this, anyway - is by phone, not email, because often you need to relay things in real time, not whenever someone gets around to checking their email. ER visit? Eh, maybe. We know she abuses the ER because she won't GET A FUCKING DOCTOR like any responsible person would.

Again, maybe. She knows how to say the right things to get attention, and most urgent care places are not set up to administer IVs for fluids and such. She's a serial abuser of the ER system, too.

I don't know, but these haydurs are even annoying me. Then a few lines about who requested the test, date and time of test, it specifies both throat and nose samples were obtained.

If a patient needs proof of a negative result, this information goes in a letter with the full details of the patient (not just DOB) which it is signed by a doctor and stamped with the clinic stamp.
