As for the most autistic I'll vote for all of the scat mods (yes, there's more than one) since it's the fetish that grosses me out the most.

On the SFW side I'll go with More Immersive Needs. There are several mods that introduce survival mechanics to the game such as needing to sleep, eat, drink and shelter yourself from the cold. Only one of them requires you to follow a balanced diet.

Besides I've never really seen the point of turning Skyrim into a survival simulator when the game world is super condensed and you're always a five minute walk from a nearby town.

On the topic of forums that makes adult content, I may as well bring up a rather obscure site known as Tessmage, a website that makes nothing but titty mods for games like Vampire the Masquerade: Bloodlines and (old) World of Warcraft. And also a VTMB fan patch that has its own history of drama and autism over modding.

For more SFW stuff since nude mods are low hanging fruit: any mod for Left 4 Dead 2 that allows things like MLP, Sonic, and FNAF. Mix those mods up with anime related shit like Neptunia and you too can have a Left 4 Dead 2 campaign of anime waifus and furries shooting down teletubbies with MLP skinned guns while fighting a tank that sounds like the great commander known as Jace Connors.

One can also include the Nexus mods site with Skyrim, especially with all its waifu companion mods that are all the same in being nothing more than near to complete glorified titty mods.

Oblivion gates in Skyrim, for some reason it was part of the Open Cities mod, a mod that made it so that cities weren't separate "zones" that the player loads into. People liked that but was mystified by the inclusion of old Oblivion gates in the cities.
People were not allowed to remove them though, because it breaks realism, Elder Scroll's lore and the snowflake creators vision where fixed Bethesda's blunder of not including them. He would sperg and send out DMCA's to anyone that uploads the mod with the gates gone, makes a mod that removes them or even writes a script that contains nothing from his mod and just flips a couple of bits and disables them.

Bethesda eventually stepped in and said that patching other people's mods were not something they would do anything about, this made some people incredibly salty.

I'm partial to mods to add ridiculous levels of details to the tiniest things, like those people who make super detailed septims and bottlecaps for Elder Scrolls and Fallout games.

A saw a 16k septim retexure once and I'm still unsure if troll or not.

The most autistic mod I can think of ever is Bob's/Angel's for Factorio. In Factorio, a game already pretty autistic to begin with, you start researching, crafting, and building machinery from raw objects. In vanilla, an electronic circuit is made of an iron plate (made by smelting iron ore) and copper wire (made from a copper plate, which is also made by smelting copper ore). But with those mods, which add additional raw materials to the game and associated recipes, to make an equivalent basic electronic circuit board, you need a "basic circuit board" (a piece of wood and copper wire), raw carbon, tinned copper wire (made with aforementioned copper wire and a tin plate, made from tin ore), resin (derived from wood), and solder plate (tin+copper+silver). There's no carbon, tin, or silver in the base game, nor can resin be extracted from wood.

I can't play Factorio without BA or PYanodons and I feel personally insulted by your comment.

There was a fairly prominent Skyrim modder who at the time had the only decent mod for expanding the civil war who had a massive spergout over Trump's election and rage deleted all his mods. I wish I screencapped it because I remember it being fairly funny.
