I've heard that either the tattoos covered up original numbers (and the skull fit over the "3" somehow) or that it's because the Spanish word for skull (calavera) starts with the letter "c" which is the third letter of the alphabet (i.e. same reason "ACAB" becomes "1312")

It's not such a stretch when you see the way his wife "cleverly" covered all his gang tattoos in photos.
