While fat fetishism has evolved into a weird brand of body positivism more recently thanks to Tumblr and other places who jumped on it years after it was established, it was originally more centered around the encumbrance and immobility and remains that way even now. Similar thing with extreme preggo, inflation, or other hyper/muscle fetishes; being too large or having body parts that are so massive that it inhibits or alters a person's ability to move properly is part of the appeal to these people and it's got really weird roots in helplessness and dependence. For most I'd consider it similar to a humiliation fetish, coupled with the inability to flee the source of humiliation. This is why a lot of fat fetishism tends to focus a lot on compulsive farting/belching/etc. in public spaces, or points out the weight gain repeatedly through dialogue intended to be read as playful teasing.
The people who are into fat fetishism from the perspective of someone who is turning another character into a fatty for the purposes of jerk fuel typically also have a high likelihood of being into kidnapping/hypnotism porn because it's a similar sort of theme; making someone dependent on someone else for their survival and treating their body as an object to be desired rather than a person with their own agency, beyond just food or cum or whatever the fuck the fetishist happens to fixate on most.

A lot of IRL fat people feel uncomfortable or bothered by fat fetishists because of this, because it's not "accepting someone as they are", with the exception of a few Tumblr people; it's about getting off on someone being in a compromised state of being and wanting to exasperate that to unhealthy degrees so they are left dependent on another person.
