I believe he was diabetic and had lost nerve sensation in the foot, then after that the blood supply to the toes failed, and the flesh died. How the maggots got in there I don't know, holes in his shoe I guess, but they kept him alive by eating the dead flesh before it could send septic sludge into the bloodstream. So the maggots got to make a home and eat his toes as more and more of his foot died. You can see that the rest of his foot is healthy red under the eaten-away parts, again because the maggots only eat decaying flesh. He must have had newly grown flies coming out of his shoes at all hours. Scary shit, eh?
