I believe I've heard of that being done as a prank or a goof on an unliked soldier, someone slips him a magazine with a blank in it so that when they do the arms drill where they cycle their weapons he unknowingly loads the blank into the breech and then this happens.

What impressed me is the guy on his dead left didn't even react in the slightest. You can't see his face very well, but everyone else around flinched or at least blinked, but homeboy just kept going on to the next movement in the drill. Dunno if that nigger should have a citation for calm in a gunfire situation or be kicked out for being completely fucking oblivious.

But it could have been far, far worse. For example, this poor motherfucker. I used to have a large hi-res version of this image, but this will have to do.
